Ok first off...damn!!! I definitely didn't see that one coming. The show runners were saying all off-season that Sara was going to be on at least 3 episodes this year. Looks like they were either saying that as a smokescreen, or Sara will show up in some flashbacks.
I was so sure Sara would die at the end of last season, but never even thought about her dying in the first episode of this season. I've got to give the show runners kudos for that.
I wouldn't go by the person in the mask in the preview, because that person was a black guy. I'm guessing it was just a League foot solder. But I could still definitely see the person that killed Sara as being Thea, Malcolm, Ras, or even Nyssa. If she was actually trying to leave the League again, I could definitely see Ras and/or Nyssa seeing it as proof she was trying to leave yet again and deciding that she needs to die. I don't know why Malcolm would want to kill Ras' daughter's girlfriend when he is trying to stay off his radar. If it was Thea, then she has totally gone off the deep end and is doing it out of spite against Oliver (and I also find it hard to believe she could've got the drop on Sara by herself with just a few months of training).

I don't think she was alone, Malcolm would never let her do something like that alone. She could have easily gotten the drop if he was walking her through it. Plus if Malcolm or Ra's would have been the one standing there Sara would have instantly been on guard. She stayed relaxed because whoever it was wasn't a threat in her mind. There was another hint there with the voice masking. I doubt they would do that for Ra's and Malcolm never used it.
Putting three arrows in that quick wouldn't be that hard for someone training day and night for 5 months.
