Makeup so that she builds up tones and features. Right now... too flat. Lose the Dillard's old lady clothes, drive to Dallas and spend a couple grand at Nordstom's.

My BIL is eye-talian. When he was younger he looked like Al Pacino. Now he looks like Abe Vigoda.

I would start with the hair. The hideous color and short cut makes her look old & sick. Then, I would change the equally hideous lip color that matches the awful hair. Darker hair with cooler highlights instead of that warm red crap, longer hair to balance out that nose, more eye definition and lighter lipstick would make her at least pleasing to look at.

I would then add highlighting to the tops of her cheekbones, temples and jawline along with contouring under her cheeks to show off her angular face.
She has a long, thin neck and nice shoulders. She can't wear a round cut neckline as it makes her head look awkward floating on top. She needs at least a slight v-neck to elongate her upper half to balance out the shoulders. Idk why she's drawn to that red when it looks so poorly on her & that small town pattern on the shirt could use a modern update.
She really has something to work with but damn she needs a stylist.
ETA No woman with a long neck should wear pearls or any necklace that hits right at the base of their throat. I don't understand why this stuff isn't obvious to everyone.
Give me 6 hours & a credit card & I could have her be a irl 6.5 and an OT 5.
