Installing AR forward assist (problem)

I installed my forward assist and it is stuck and will not function. I took it out several times and I even tried several different roll pins. It is acting as if the roll pin is too wide. I used a punch to hold the FA in which is a little smaller than the roll pin and the forward assist worked fine. The FA looks normal but it gets stuck and will not move once the roll pin is all the way in.

Has any one else had this problem and if so, how did you correct it?
