Kentucky came very close just two years ago. Just lost on a last second 3 and in the SEC Tournament Championship. So maybe it is more possible than it seems.

Their second loss was in the national championship game. They lost the last Sunday of the regular season on a buzzer beater.

The closest that come to mind are the 2005 Illinois squad, the 1999 Duke squad (both lost in the championship), and the 2011 UK squad.

I think the 2004 St Joe's team with Delonte West and Jameer Nelson was undefeated in the regular season, I believe. They lost in their conference tourney, if I'm not mistaken and then in the Elite 8.

Undefeated heading into the tournament is doable if you're a mid-major power, but after that, it is damn near impossible.

Eta: UNLV in 1991 was undefeated before the final 4, but they were also kind of a mid major.
