Will you kindly help clean up a logo?

If someone has the time, I would greatly appreciate it if you would help clean up my daughter's softball team logo. It has been through the ringer, and no one has access to the original file. I do have photoshop CS6, but I keep fricking up the image. Thank you oh great wizards and magicians of photoshop

I'd like to be able to use the graphic for the team website, and to have some memorabilia created for my daughter. It looks like the logo has been saved, resized (many times), and the colors look like they have washed.
I've figured out how to recreate the text (font, size, color, etc), it is just the logo which I'd like to get "cleaned".

PHOTOSHOP:
1) Remove the text at the top (simple font that can be recreated)
2) Color Select all yellow and remove (cleaner looking to recreate the yellow)
3) Clean up any major artifacts, color bleeds, etc.
ILLUSTRATOR:
1) New document
2) Place image cleaned up in Ps into document
3) Live Trace as One Color Logo - this creates a vector image from the raster image
4) Resize as needed
5) Export to a png file
