Bela Lugosi died while filming some crappy movie. See Ed Wood if you haven't already. Also Chris Farley was originally cast as Shrek and recorded a good bit of his lines but not enough to finish the movie.

Lots. The most famous was James Dean, who died during Giant. His scenes were already filmed, but there was post-production work to be done and some of his lines were dubbed by Nick Adams.

Natalie Wood, Vic Morrow, Tyrone Power and John Candy were some other notables.

"Coach" dies before the end of the third(?) season of Cheers. Kafka is probably the only other one who knows about "Merrill's Marauders", the lead actor died before the movie was released.

Bea Benaderet (Kate on Petticoat Junction) passed away while the show was still in production. They wrote her out of the series, saying that she was leaving to take care of other relatives. When they wanted to write her back into episodes as a guest star, they would bring in a lookalike and shoot her from the back and dub in Bea's old audio tracks.

Someone would get married or have a baby, and they'd say, "Kate's here!", show the other actress from the back for a quick scene, then move on.
