Which city has the best surrounding areas for fun outdoors? Specifically fishing, boating, and hunting; turkey, ducks and deer (in that order).

Might be the only thing on the planet that Shreveport wins at....
Birmingham probably wins but it comes with an asterisk. Since it is bigger than the other cities, you have to drive a little farther to get to these things.

Lafayette is by far the best city in the state of Louisiana. Just basically cut anything that is 30 min or more north of I-10 from your search.

What kind of fishing? I'd go ahead and cross Jackson off your list and get that out of the way.
Best Duck hunting would be within an hour of Shreveport.
Best Deer hunting and bass fishing would be within an hour of Birmingham
Lafayette is the only one on your list that offers close access to saltwater fishing.
Birmingham offers the best outdoor scenery from that group by a landslide. Birmingham is going to be at or near the top in everything you mentioned but will be a barren wasteland for a man after ducks compared to the others listed. There are multiple world class bass fishing lakes that are a reasonable drive from Birmingham, and even streams nearby that hold rainbow trout

Mississippi River and oxbow lakes are closer to one of these than any other.
You know the answer but to avoid the problems in Jackson plus have a good spot for Turkey, deer, and duck it's gonna cost you

I can't speak for Madison or Birmingham. See below for my experience in Lafayette and Shreveport.
Lafayette: Lots of freshwater fishing within a short drive of town. Henderson and the Atchafalaya Basin 30 minutes to the east. The fishing here is dependent on the river stage but can be very good. Lacassine Pool about 1.5 hours to the west. No too terribly far a drive from Old River and other lakes up North. No great saltwater fishing in the immediate area, Vermillion Bay can be very good but is seasonal. Still within day trip distance of Big Lake and the Houma area. In short, if you like to fish, there will be plenty to keep you busy. Public land for hunting is limited seeing that you are in a very crowded part of the state. The Atchafalaya basin has lots of public land, but it can be crowded and you typically need a boat or four wheeler. Large deer are killed every year on public land in the basin. Public duck hunting in the basin is very hit or miss and can be crowded. Excellent private land duck hunting to the southwest in Vermillion and Cameron parish if you have connections and deep pockets. The turkey hunting in the basin used to be great, the flood in 2011 pretty much wiped that out. The best public turkey hunting in the state is within day trip distance on Kisatchie NF. While I have spent most of my outdoor career in this area, it is not near the top of my list for hunting.
Shreveport: I will preface this by saying that I have not spent as much time in this part of the state as I have around Lafayette. There are plenty of freshwater fishing opportunities in NWLA. Some of the larger reservoirs have invasive aquatics issues, but people still catch lots of fish up here. I've lucked into some fantastic fishing in/around the Red River. As far as hunting goes, there is a good bit of public land between the red river, state WMA's, and Kisatchie to the east. My experience in NWLA has shown me that this part of the state is not a top notch waterfowling area, but you can kill a few ducks if you are willing to do your homework. There is some good private land duck hunting across the border in Arkansas. The piney woods to the east has some turkeys and public land to hunt them on.
I would probably prefer to live in the Lafayette area, mostly because I feel I could I could at least match the success I would have hunting on public land around Shreveport, plus I would have access to saltwater fishing.

One of my favorite places near Huntsville is the Joe Wheeler Widelife Refuge. There's also plenty of places to fish as well, river/lakes/ponds. I think a lot of people underrate Huntsville. It's got a lot to offer in those regards and is also close to the smokies as well.

I don't know about the others, but can tell you about S'port and Lafayette. IMO, Lafayette is definitely better for fishing. You've got plenty of freshwater options as well as salt water. I know of a few decent duck hunting spots within an hour. I spend my winters deer hunting around Shreveport, but if that's at the bottom of your list, I'd go with Lafayette.

Pretty decent duck hunting about an hour or so north around Guntersville

Let's just remove Jackson from the conversation.
Bass fishing: Shreveport and Birmingham are 2 of the best places in the country. Laffy is good for quantity but really quality fish aren't available in huge numbers.
Saltwater fishing: Laffy is head and shoulders above the rest, but Shreveport is only 3 hours from big lake.
Boating: I imagine you can find places to boat around any of them
Turkey: Laffy is at the bottom. Shreveport and Bham have opportunities within an hour.
Ducks: Laffy waaay above Shreveport. Shreveport is ringnecks and gadwall and woodies. And looooots of people. If you have some $$$, hunting up in AR is close enough and good. And if you're willing to drive 2hrs+, the Monroe area is available but again very crowded. Enough big lakes around B-ham that I imagine you can kill a few birds but it isn't known for it.
Deer: There is some awesome public land deer hunting near Shreveport. Big deer too. In my experience in south LA, public land deer opportunities are more crowded than north LA and big bucks are harder to come by. There are lots of deer near B-ham but I know nothing of their public land possibilities.

I grew up right next to Wheeler Wildlife Refuge, I know it pretty damn well..Strike that.. I know it like the back of my hand. Even so,there is so much other cool shite so close by,Hour or less drive.
Want to get lost in the woods on your 4 wheeler,go to Bull Run in Jackson County, want to do some canoeing with some mild rapids? Elk River in TN.
Hunting,it's all around you,same with freshwater fishing. Want to jump off some cliffs into deep cold water? Lay around on a waterfall and drink beer? Hunt arrowheads?
