Tell us your story of beating contraceptives?

Buddy is going to wedding this weekend and couple has a 2 yo together, unplanned.
Got me thinking, anyone else have an unplanned child while using contraceptives? If yes I'd be curious to know what you were doing and if you have an idea of what may have happened to affect the contraceptives, e.g. medication reaction, ripped rubber, didn't pull out...
I've never been one for condoms, so it's always been woman on pill and me pulling out, has anyone specifically gotten a woman pregnant employing both?

I've had a doctor tell me the only way the pill can fail is if the girl isn't taking it every day as directed and that's why it has a 99% success rate to account for human error. Not sure if he was full of shite but I take my chances with it.

Oh really? I feel like the success rate would be higher to account for human error.
