Have we had our first reality series death yet?

I don't mean after-the-fact that such-and-such died a year after filming, I mean someone who literally died in the middle of one of these things.
I'm guessing this is more likely in some other country like Japan or Russia where they give no-fricks but what about a US-based show?
I remember some guy passing out and falling into the burning coals on survivor some years back forced them to stop filming and fly the guy to the ER but i haven't kept up much so i'm wondering if an actual death has occurred during filming yet?

Someone died during that Everest documentary, and captain Phil died on deadliest catch.

Someone died during Israeli Survivor.
It's never happened in US Survivor but there have been some close calls. Usually because of guys being dehydrated and then overworking themselves during challenges.
Fun fact the guy you mentioned in your OP came back years later and played another season. He's now in jail for possession of child porn

There was that one on VH1 where the contestant murdered a chick in Vegas. Happened while the show was on air, but that may not qualify for what you are asking since it didn't happen on the show.

I have no clue how someone hasn't died on Biggest Loser.
They take people with massive health issues and then kill them with working out all day, every day.

That one chick probably should have died when she rocketed that watermelon into her own face at point blank range with that giant slingshot.

Surely someone has died during the year that a 600 lb life does the documentary. Doubt they would air or even make it public if that happened though.been a few close calls.
