My theory is that Ned and his men meet and fight the Kingsguard at TOJ, just like Ned remembers, not yet knowing the truth about R and L. "No, now it ends."

But his recollection is that as the fighting begins, he hears L calling out for him from the tower, then his vision jumps right to him at her bedside.

My guess is that Howland slips away during the fight to rescue her as the crannogmen are stealthy and likely not much help against the likes of Dayne et al.

There he finds her and she pleads with him to stop Ned because they were actually protecting her.

When everyone is dead except for Ned and Dayne, Dayne clearly bests Ned and is about to kill him when Howland runs out and stops him explaining that they both want to protect L and her baby, hence, saving Ned's life.

Ned rushes in to see L where she explains that her and R are in love and married, then pleads with him on her deathbed to work with Dayne to protect JS......."promise me"
