Question about time out length

What determines if a time out is 30 seconds or regular length? I can't seem to figure out a pattern.

re: Question about time out length
In the Denver game, I heard the referee say 20 second time out. What's the time for a regular time out?

re: Question about time out length
quote:

On charged team time outs, the Field Judge starts watch and blows whistle after 1 minute 50 seconds, unless television does not utilize the time for commercial. In this case the length of the time out is reduced to 40 seconds.
