Do you mean a lease template or are you referring to some type of insurance policy?

I own a Condo that I rent out. I just found out the homeowners policy is not the correct type of policy to have on the property as a landlord. The company I have the homeowners policy with doesn't write landlord policies in Louisiana.

I use a million dollar umbrella policy. It covers my entire family and my condo renters. They are pretty inexpensive and are a good peace of mind. If you or someone in your family were at fault in a wreck and caused someone to be disabled, it would certainly cover more than the standard liability medical would.

Use an Independent Agent to find one that fits your need. We used Sheri at The Fourrier Agency. LINK

She was very efficient and easy to deal with.

Tenant occupied policies for Condos are not as easy to find. There are a few carriers that I know of. Gulfstream is one. And maybe UPC. Not positive on UPC.
