I don't know what happened and it appears that a human error was made. So having a tool in place to stop that human error is "SEMS" which is what I said sarcastically. Our government has mandated that everyone use this BS which is what it is. I know a few things about SEMS I was part of the committee that wrote RP75. When RP 75 was written it was only intended to be a recommended practice.

Latest report I've seen tonight is two are missing none confimed dead. There are tools that can easily prevent this. When employees become expendable, the engineering solutions take a back seat

SEMS is essentially a documententation exercise that companies can use towards the goal of continuous improvement. Yeah, it requires the use of a JSA, Haz ID, Management of Change, Stop Work, etc, etc, etc......but one thing it does not manage is the human element. You can create the perfect plan (SEMS) but that is all it is - a plan. Nothing goes by plan, and sometimes in life accidents happen.
Too many times I hear the term "Safety Culture" thrown around as if offshore employees are cowboys, and it amazes me b/c in my brief 10-year career, I have yet to find anyone that is willing to risk their life for the advancement of their company. We all want to go home to our families when our tour is over.
Yet, we have all of these Government agencies setting up strike forces and investigation teams....putting out innacurate data about two deaths....etc, etc, etc.....and they will undoubtedly come to a similiar conclusion - more regulations (magical tool) would have prevented this.

Apparently people just don't get that no tools can stop a human error. It is sad that two men are still missing and will likely never go home to their families. And it is likely because of a human error.

bull shite. people are always taking shortcuts and sometimes it costs them. these smaller companies are outright dangerous alot of the time. they get bottom of the barrel contract employees for as cheap as possible and they pretty much have zero safety program in place.
