I remeber seeing an actor talk about this, dont remember who, on Inside the Actors Studio. I believe theres a tuck method they use. I know for nude scenes they wear some sort of nude underwear.

I read or heard something one time Ewan McGregor was filming a sex scene in a movie and had an "accident". They never said what it was but they way they were hinting was that he by accident inserted himself into the actress.

There's a snopes discussion board saying this; however, he's only admitted to getting a boner while filming his first ever sex scene as an actor with Alice Krige (played Borg Queen in First Contact - Christian Bale's mom at the beginning of Reign of Fire)

I've heard Courtney Cox got accidentally stabbed in "3000 miles to Graceland", and Donald Southerland and some actress did it (not on accident) in the 70's while shooting some movie.
