Put an order in for one in December after one of you guys brought it up. Finally received an email from them saying it would be March or April before it ships.

No, the form was available on the website. They have since removed it. After all the hysteria.

It was 1995.00. Don't know if they are still taking orders. I probably still have the blank form if you need it.
