That part of The Shining to me, as a kid. 
The Exorcist scared me so much I refused to sleep in my room upstairs and slept on the couch until I was 10 years old.

In college, I read Dave Cullen's "Columbine," an incredibly detailed retelling of the entire Columbine tragedy; right down to the psychology of the two killers, the befores and afters, the whole schabang.
Delving that deep into the mind of Eric Harris fricked with me pretty bad, especially when it was revealed that his favorite movie of all time was Event Horizon. I think watching that movie and always having that connection with those murders has made it a very fricked up film for me.

Aliens certainly fricked with me as a little kid. I had a recurring nightmare that one of the xenomorphs would be lying on the top of my headboard or at the end of the hall way, in front of my parents' room, waiting for me to get scared and run to their room.

Forgot about Jaws...Jaws fricked me up as a kid, and if I'm being honest, for life really. As a kid, I used to get pretty freaked out at night in a swimming pool because of that movie. Even though I knew I was in a pool, the thought of being in water and not being top of the food chain made me completely uneasy. And in the ocean is much, much worse (though I do love being in teh ocean).

Also Alien gave me nightmares as a kid. I wasn't so much scared of the movie as I was of the dreams I used to have. shite was weird.
This part of The Shining always freaked me out pretty good:
And honestly, it still kinda does. Just so weird and random and WTFish. Freaky shite.

I remember watching Fire in the Sky and not getting a good nights sleep for days afterward. I must have been 12 years old.
I remember seeing Event Horizon in high school and I have never had the desire to see it again.
Ghost movies don't bother me, nor do demons etc. But the alien/space genre of horror gets me every time

Have you ever seen Sunshine? It's VERY similar to EH (though EH is better). Both movies fall off a little in the final acts, but both are very solid. EH is fricking great.

Different time but...
My only answer is the original Halloween
To see that in the theatre at age 11 (like I said way diff time) I was fricked up
It been so bastardized over the years that its style is nothing new to anyone
But the music the initial view from the killer perspective the normal-ness of the setting
Those things were all "new" at the time
Truly terrified at the time and I never trick or treated again
I've seen almost all the others mentioned and none had the same effect
Exorcist It hills none of those did what Halloween did

Looking back, I can't believe my parents let me watch that, yet wanted to cover my eyes in Raiders of the Lost Ark when the nazi's faces melted.
Another one that freaked me out was Red Dragon. Ralph Fiennes' performance was so ridiculously convincing, I remember being scared walking out of the theatre with my GF (pimp).
