Without observing and and accounting for every calorie consumed and burned over a long period of time, it's just an assumption. Most people fall within the mean and have a low BMR variance...think 100-250 calories between similar ages and statures....
I'm sure you have treated non-obese people but the vast majority of lean people are healthier and at less risk of chronic disease than the obese....
Being overweight/obese is most of the puzzle, not just one piece unfortunately. #1 indicator of present and future chronic disease by a long shot....
