San Francisco health officials say a program that provides alcohol to homeless alcoholics prevents the city's most vulnerable from using emergency services

Experts say the program can save or extend lives, but critics wonder if the government would be better off funding treatment and sobriety programs instead.

"Established in countries such as Canada and Australia, a managed alcohol program is usually administered by a nurse and trained support staff in a facility such as a homeless shelter or a transitional or permanent home, and is one method to minimize harm for those with alcohol use disorder," the California Health Care Foundation explains in an 2020 article describing the pilot program.

"By prescribing limited quantities of alcohol, the model aims to prevent potentially life-threatening effects of alcohol withdrawal, such as seizures and injuries."

The San Francisco managed alcohol program, or MAP, was established during the COVID-19 pandemic to prevent vulnerable homeless people who were placed in isolation in hotel rooms from suffering from alcohol withdrawal. But the program, which started with 10 beds, has since been expanded into a 20-bed program that operates out of a former hotel in Tenderloin with a $5 million annual budget, the San Francisco Chronicle reported.

Alice Moughamian, the Nurse Manger of the Managed Alcohol Program and the San Francisco Sobering Center, explained in an October presentation that nurses provide clients with a motel room, three meals a day, and enough alcohol "to meet their addiction needs, but keeping someone at a safe level of intoxication."

Initial success in stabilizing alcoholic patients prompted health officials to expand the pilot into a long-term program with 10 beds earmarked for "the Latinx and indigenous population," while 12 additional beds are supported at the city's traditional sobering center, Moughamian said.

Bryce Bridge, a social worker involved with the program, said during the presentation that once a client is identified as having alcohol abuse problems and admitted, they are assessed to determine individual needs. Clients are connected to a primary care doctor, provided resources to secure government identification if they lack a social security card or other documents and assisted with psychiatric care, wellness activities on site and other evidence-based treatments.



FYI, In Louisiana, they train people in sober living facilities how to abuse Medicare. I know for a fact that if someone in a sober living facility wants a ride from say Lake Charles to Baton Rouge then Medicare will pay for an ambulance to bring them even if it is not an emergency. Many abuse this to just go back home instead of buying a bus ticket.

Not ideal but cheaper than an ED visit or ICU stay for DTs.

San Fran also has tax payer funded places where you can go to clean needles and they will help you to shoot up your drugs. The idea is to stop the transmission of disease. They also hand out Narcan. I say we should let the weak kill themselves off and stop paying to coddle their poor lifestyle. Stop all of these services and tell them all tough shite.

Have you ever been to a sober living home? I have and it was complete sobriety

Do you know what is alcohol withdrawals and how it can be lethal?

frick SanFran but I somewhat know where there're coming from as an alcoholic

Not because I am not an alcoholic. My only experience is hiring people from there for work. I hire people from one not far from me regularly so I have heard and seen a ton of shite from people with various addictions.

Yes alcohol withdrawals are awful and can be deadly. I’m ok with a little alcohol provided to wean off. That’s the idea behind suboxone for opiates, I think. But something tells me this Tenderloin place is not really doing shite. Mostly I despise the open racism. “Latinx only” beds.

It's a tapering down method which works, especially in my experience. I was in rehab with a guy ~50ish and stopped drinking cold turkey. When he did he had 2 heart atacks suddenly.

It probably encourages abuse more than helps it. Just like their needle program. Sometimes people have to hit rock bottom and yes that may include dying from abusing their bodies.

If this was a program to inject fat people with sugar who were having diabetes-induced seizures from withdrawals then I am 100% sure the few people here sympathizing with it would change their tune and say shite like "change your lifestyle" "lose weight" "its their own fault" etc

Glad it worked for you but over 90% of alcoholics relapse. It's pretty irritating to keep helping people who are hell bent on destroying their lives. It gets especially irritating when I pay my health insurance knowing I am subsidizing this sort of abuse thanks to Obamacare. I say this as someone who had an alcoholic family member

You are sympathetic due to personal experiences but I bet you feel differently about drug users, overweight people, etc

I worked in SF for many years. Back in the 70s and 80s it was a world-class city. It was governed by traditional old school Democrats like George Moscone, Frank Jordan, Diane Feinstein and Willie Brown. They loved The City. Then the Progressives took over and destroyed it with benign neglect. Sad.

Alcoholic here as well. I have a soft spot for these folks. If you go through alcohol but specifically vodka withdrawls after years of abuse it’s devastating and very dangerous. They probably saved money in the short term.

It’s incredible because you know exactly what’s heading your way and yet you still do it. I think the number is closer to 95% personally haha.
